The sustain pedal, CC 64, reaches nothing in the engine: midi/router.h's Controller keeps RPN (101/100/6/38), NRPN's flags and all-off (120/123) and drops every other controller, and the plugin's MIDI dialect handles pitch bend only. The soundboard and damper-pedal patches put their pedal on macro 1 (a host parameter named "Pedal"), so a DAW has to map CC 64 to the plugin's first parameter by hand, and the workstation has no way to do it at all.

What to decide: whether CC 64 maps to a macro by convention (a fixed macro, or a per-patch assignment), whether it is a switch (>= 64) or continuous, and where the mapping lives -- the router, the instrument, or the project's inputs. Related to #48.
